Testosterone Gel/Cream for Men

What is Testosterone?

Testosterone gel or cream is a topical medication applied to the skin to raise low testosterone levels. It’s used once or twice daily and helps improve energy, mood, libido, and muscle mass

  • Your doctor has prescribed testosterone therapy.
  • These hormones are bioidentical—designed to match what your body naturally makes—and are commonly used to treat low testosterone.
  • In men with low testosterone, restoring levels may be associated with improvements in energy, mood, muscle mass, sexual health, and general quality of life.

Within a few weeks to months, you may notice:

  • More energy and better mood
  • Improved focus and motivation
  • Increased sex drive and responsiveness
  • Early strength and performance gains when exercising

Results vary—sexual and erectile improvements may take 3–6 months.

With continued use, testosterone therapy may:

  • Build lean muscle and reduce fat
  • Protect the heart and blood vessels
  • Stabilize mood and confidence
  • Support brain function and memory
  • Improve blood sugar and cholesterol
  • Reduce risk of chronic conditions like diabetes and Alzheimer’s

Your doctor may order blood tests to monitor your progress. Key points:

  • Lab testing is usually done at baseline, 1–3 months in, and then every 3-6 months.
  • Avoid drawing blood from areas where cream was applied
  • Common labs: Total & Free Testosterone, Estradiol, PSA, SHBG, Hematocrit, DHT, FSH/LH
  • Avoid ejaculation for 48 hours before a PSA test

Most men tolerate testosterone well. Side effects, when they occur, are often dose-related and manageable:

Common:

  • Oily skin or acne
  • Hair thinning (if prone to baldness)
  • Mood changes or irritability
  • Mild swelling or fluid retention
  • Breast tenderness
  • Reduced sperm count and testicle size (may use HCG to help)

Less Common or Serious:

  • High red blood cell count (may need to donate blood and hydrate more)
  • Increased blood pressure
  • Worsening of untreated sleep apnea
  • Rare liver strain or prostate changes
  • Signs of too much estrogen (e.g. breast growth or emotional swings)
  • Chest pain or trouble breathing
  • Vision changes or severe headaches
  • Swelling in legs or unusual bleeding

⚠️ Testosterone Black Box Warning

1. Secondary Exposure Risk (Topical Products)

  • Children and women can be harmed if they come into contact with skin where testosterone gel or cream has been applied.
  • Can cause early puberty in children and signs of virilization (e.g., hair growth, acne) in women.
  • Users must wash hands after application and cover the application site.

2. Blood Pressure Increase (Injectable Forms)

  • Certain testosterone products (e.g., testosterone undecanoate like Aveed) carry a risk of serious blood pressure increases, which may raise the risk of heart attack, stroke, or death.
  • Monitor blood pressure regularly during treatment.
  • If you have sleep apnea, talk with your doctor. Sometimes, sleep apnea has gotten worse in people using testosterone.
  • If you’re trying to have children, talk to your doctor—testosterone lowers sperm count
