Thyroid (T4 & T3)

What are thyroid hormones?

Thyroid hormones (T4 and T3) are hormones made by the thyroid gland that regulate metabolism, energy, body temperature, heart rate, and brain function. T4 is the inactive form, and T3 is the active form used by cells.

You were prescribed thyroid hormone replacement—either a compounded T4/T3 combination or porcine (desiccated) thyroid. These medications help restore thyroid hormone levels when your body is not producing enough. Porcine thyroid contains both T4, T3, and trace thyroid hormones (T2, T1), mimicking the natural thyroid gland.

Many patients notice:

  • More energy and less fatigue
  • Improved mood and mental clarity
  • Better temperature regulation (less cold intolerance)
  • Reduced joint/muscle aches and improved comfort
  • Regulated menstrual cycles and reduced PMS

With regular use, thyrooid may help with the following:

  • Healthier metabolism and easier weight management
  • Hair regrowth and improved skin texture
  • Stabilized mood and cognitive function
  • Reduced cholesterol levels
  • Better hormonal balance, especially when combined with other therapies
  • Initial labs about 6–8 weeks after starting or adjusting therapy
  • Regular monitoring every 6–12 months once stable
  • Take your blood test before your daily dose of thyroid on the morning of the test.
  • If you’ve already taken your dose that day, wait at least 6 hours before drawing blood to avoid a temporary spike in T3/T4 levels that can affect results.
  • Common labs: TSH, Total & Free T3, Total & Free T4, and optionally Reverse T3, TPO and Ferritin
  • More frequent monitoring may be needed during pregnancy or with autoimmune thyroid disorders
  • Too much thyroid may cause:
    • Fast heartbeat or palpitations
    • Shakiness, anxiety, or irritability
    • Sweating or heat intolerance
    • Headache, diarrhea, or weight loss
  • Too little thyroid may still cause fatigue, depression, hair loss, or cold intolerance
  • Hair changes or loss can occur during dose adjustments
  • Rapid or irregular heartbeat
  • Chest pain or shortness of breath
  • Severe anxiety, tremors, or insomnia
  • Worsening symptoms or new onset of symptoms after dose changes
  • If you are pregnant or planning pregnancy
  • Notify your provider if you have heart issues, are pregnant, or have adrenal dysfunction
  • Do not switch between brands or forms without guidance—potency varies
  • Thyroid needs often increase in pregnancy. Tell your doctor immediately if you’re pregnant or planning to become pregnant.
  • Thyroid hormone can increase heart rate and blood pressure—report any racing heartbeat, chest pain, or shortness of breath right away.

Thyroid hormones should not be used for weight loss or obesity treatment.

In euthyroid (normal thyroid) patients, doses within the normal range are ineffective for weight loss, and larger doses can cause serious or life-threatening effects, including:

  • Heart attack (myocardial infarction)
  • Heart rhythm disturbances (arrhythmias)
  • Cardiovascular collapse
  • Take on an empty stomach with water—30–60 minutes before food
  • Avoid calcium, iron, coffee, antacids, or fiber within 3–4 hours of your dose
  • Take consistently at the same time each day
