Estrogen Vaginal (Estriol or Estradiol)

What are Vaginal Estrogens?

Vaginal estrogens are low-dose estrogen medications applied inside the vagina to treat local menopausal symptoms like dryness, burning, itching, and painful intercourse. They come as creams, tablets, or suppositories.

Your doctor has prescribed vaginal estrogen therapy (either estradiol or estriol) as a cream or suppository. These are bioidentical hormones designed to help restore vaginal and urinary health by replacing the estrogen your body no longer makes after menopause.

Most patients notice relief within a few weeks, including:

  • Less vaginal dryness or irritation
  • Reduced pain during intercourse
  • Fewer urinary issues (urgency, burning, leakage)
  • Improved comfort and overall vaginal health

With regular use, vaginal estrogen may:

  • Restore healthy vaginal tissue and pH
  • Prevent recurring urinary tract infections
  • Support bladder and pelvic health
  • Improve overall quality of life with minimal systemic hormone exposure
  • Routine labs are usually not needed for vaginal estrogen.
  • Continue with yearly Pap smears and breast exams as recommended by your doctor.

Most women tolerate treatment well. Possible side effects may include:

  • Mild vaginal irritation, discharge, or spotting
  • Breast tenderness or bloating
  • Headaches or mood changes
  • Skin changes at the application site
  • Unusual or heavy vaginal bleeding
  • Pelvic pain
  • Signs of infection (odor, itching, or burning)
  • Yellowing of eyes or skin, or swelling in hands/feet

Let your provider know if you have a history of breast cancer, unexplained vaginal bleeding, or are pregnant or breastfeeding

  • Use as indicated by your doctor.
  • Use at bedtime to help the medicine stay in place
  • Avoid sex immediately after application to reduce transfer to your partner
  • Wash hands well after use
  • Do not use tampons while on vaginal estrogen
  • Store suppositories in the fridge so they don’t melt
